First Visit
It is such a pleasure to serve you as our new patient.
Your first visit to our office, referred to as “consultation” or “exam” appointment, will give us the chance to get to know you and your main concern about your smile. In return we provide you with the diagnosis and treatment plan options that may address your concern at best.
Recommended considerations in your consultation appointment:
- A parent or guardian must accompany all patients under 18 at the consultation visit.
- We provide same day dental services. In case of a need for a medical release from your physician due to a complex medical history that we may require a second appointment to initiate your treatment
- In case of any medical condition (including but not limited to diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, …) or if you require taking Antibiotics prior to dental procedures, and / or you are on any medication.
- It would be beneficial letting us know about your medical and dental history, or any dental insurance information in your consultation appointment. To save time you can even provide us with the mentioned information through filling out forms online prior to your consultation appointment.
